New Patient Registration
If you are new to the area and wish to register with one of our GP’s please ask at our reception. If you have your medical card then please bring this along with you.
Following your registration our practice policy states that you will be required to have a new patient check to enable your registration to be fully complete. An appointment will be offered with our health care support worker during your registration. Please note, it is important that you attend this appointment in order for our staff to be able to offer you the highest standard of treatment.
Individuals wishing to register with our Surgery should note that you will be asked to agree to some terms and conditions of registration, as shown below:
- Abuse towards any member of staff will not be tolerated and will result in immediate removal of registration
- This practice does not prescribe benzodiazepines or sleeping tablets
- Patients will not be offered home visits unless they are housebound or a GP deems it necessary
- You should always give notice if you wish to cancel an appointment or cannot attend
- Patients must try to attend appointments on time
Please note that persistent failure to attend appointments may result in you being removed from our patient list.
